I have the same problem. I am getting "Bad request from NAS" message
from the Cisco ACS 3.1 server.

My wpa_supplicant configuration is:
============ start
ctrl_interface=/var/run/wpa_supplicant
ctrl_interface_group=root

eapol_version=2
ap_scan=0
network={
   ssid=""
   key_mgmt=IEEE8021X
   eap=PEAP
   identity="jnz\jacky"
   password="jacky"
   phase1="peaplabel=0 peapver=1"
   phase2="auth=MSCHAPV2"
}
======== end

And my hostapd config is:
======== start
interface=eth0
driver=wired
logger_stdout=-1
logger_stdout_level=4
debug=4
dump_file=/tmp/hostapd.dump

ctrl_interface=/var/run/hostapd
ctrl_interface_group=jacky

auth_algs=3

ieee8021x=1
eap_message=hello
use_pae_group_addr=1
eap_server=0
nas_identifier=test

auth_server_addr=192.168.2.142
auth_server_port=1812
auth_server_shared_secret=cisco
=========== end

hostapd log is
